Narrative

Employee was dropping the inspection door over the head gate sprocket and lost control of the door when EE attempted to drop the cover down causing EE's right hand and EE's middle finger to become caught between the door and the ground. Compliance with rules/regs, PPE, mining equip/systems not a factor. Miner proficiency may be a factor.
