An unintentional roof fall was discovered on 11 Zone panel in the #10 entry at 6 block. This fall measured 8'H X 20""W X 20'L. No injuries occurred with no damage to any equipment, ventilation controls, or escapeways. All approaches have been dangered off with additional roof support set. Reference # 03607983.
