Narrative

On 2/4/23 an employee turned on a fan after shutting it off to use the underground phone. Upon starting fan backup some dust came out of the fan where EE felt a foreign body enter right eye. EE flushed eye & didn't report incident to supervisor that shift. 2/5/23, EE's eye was irritated so EE went to physician & had metal flake removed & was given restrictions.
