Narrative

Employee was removing a bolt that was helping secure a scoop bucket to an equipment dolly. As employee was removing the bolt the scoop bucket shifted pinching employee's right index finger between the bolt and bucket. Employee received an avulsion to the finger. Employee was wearing appropriate PPE and has training and experience.
