Narrative

EE was walking down #4 entry to get a roll of fly pad. EE bent over to pick up the fly pad, draw rock fell hitting EE in head and back & knocking EE into the muddy floor. Rock measured 12"" wide x 24"" long X 11/2"" thick, EE was taken to hospital and found to have no broken bones but was taken off work until seen by specialist
