Narrative

Employee was loading a leg jack onto the flex pan on a dolly. A come-a-long was being used to remove the jack from the scoop when the jack fell from the scoop bill, striking the employee in the lower right leg. Compliance with rules/regs, PPE, mining equip/systems not a factor. Miner proficiency may be a factor.
