## Narrative

While pulling out a rock in a jam in conveyor #2; bar slipped and hit me on the head.  This caused employee to fall and hit shoulder on side of plant (eye beam). At first, thought to be muscle sprain.  Employee stated that pain got worse, was sent for an x ray.  Found employee had neck injuries (pre-existing).  We question whether this happened or not.  No one saw employee fall.
