Narrative

Employee was operating a golf car on Main East Haulage at 11-30 when employee's head came into contact with the roof knocking off employee's hard hat and causing a scalp avulsion. Compliance with rules/regs, PPE mining equip/systems not a factor. Miner proficiency may be a factor.
