Employee was working on the surface area of an underground mine when employee tripped over a 2"" hose and fell. This incident did not involve any aspect of compliance with rules and regulations; mine equipment or mining system; job skills and miner proficiency, training and attitude; or protective items relating to clothing, or protective devices on equipment.
