Narrative

The Employee was bolting in the #5 face on the #2 section when a rock measuring 10""x6""x1/4"" fell from the top striking the employee in EE's hard hat causing a laceration to the left side of EE's neck and ear. The entry was approximately 74"" high and damp. The employee received 5 sutures.
