Narrative

Employee was bolting in 3 Left crosscut and had just finished drilling the hole and was in the process to install a bolt. As they pushed the install iii bolt up using the fast feed it stopped and they bumped the rotation. When they did it caused the 24"" bolt wrench to break and strike them in the side of the face. Employee received 12 stitches.
