Narrative

The EE was in 9 North Mains Section, bolting in the ladders on the left side. The injured was installing 16' cable bolts. The hole was drilled, and the starter steel became stuck. The EE reached for another steel, and when EE turned around the starter steel fell from the hole and struck EE in the head. EE received 3 staples in the head, but didn't miss any work.
