Narrative

The injured EE was tubing on the 5W Section. The injured was standing back by the FCT, when a piece of head coal and rock fell from the roof and struck the EE on top of the head. The piece was approximately 2'x2'x5"" thick. The EE briefly lost consciousness. EE was treated and released from the hospital on 9/11/23. Began missing 9/12/23.
