Narrative

Employee was dropping in pan at the shaft and was guiding the pan down that was hooked to the hoist on top of another piece of pan. As it sat down it lost tension from the hoist and slid back, pinning EE's arm between the pan. Compliance with rules/regs, PPE, mining equip/systems not a factor. Miner proficiency may be a factor.
