Narrative

The Employee Knelt down to change the water filter on the RCM SN JM7225 in the #5 entry on 2 section. Employee had left hand on top of the miner when a rock measuring approximately 12"" by 9"" by 7"" fell striking the employee's left pinky finger. The area was approximately 6 feet high. This incident resulted in a partial amputation.
