Narrative

Employee was attempting to install a roof bolt in #2 entry when a piece of stone fell from roof between straps hitting employee in the knee. The cause of this injury was improper scaling of the roof. PPE was not a factor, mining systems was not a factor, compliance with rules and regs was not a factor. Miner proficiency was a factor.
