## Narrative

Employee was attempting to fill up the water car. The EE employee tripped on the water fill line and fell down. The cause of the incident was improper body mechanics. PPE was not a factor, mining systems was not a factor, compliance with rules and regs was not a factor, miner proficiency was a factor.
