Narrative

Employee was attempting to install a rib pin. The EE got hand caught between rib and bolt wrench. The cause of this incident was improper body mechanics. PPE was not a factor, mining systems was not a factor, compliance with rules and regs was not a factor, miner proficiency was a factor.
