Narrative

Employee was attempting to walk around the tailpiece. The EE slipped and fell on EE's left forearm. The cause of this incident was improper body mechanics. PPE was not a factor, mining systems was not a factor, compliance with rules and regs was not a factor, miner proficiency was a factor.
