Narrative

Employee was attempting to pick up a piece of eicho track. The EE hands slipped and the eicho track landed on employees foot. The cause of this incident was improper body mechanics. PPE was not a factor, mining systems was not a factor, compliance with rules and regs was not a factor, miner proficiency was a factor.
