Narrative

Employee was drilling 1-5/8"" holes and when in the process of changing out the drill bits, the head released and pinned EE's Right-Hand little fingertip between the Jacobs chuck and the plate EE was drilling. This caused a partial amputation of the Right-Hand little fingertip.
