Narrative

Employee was attempting to change a vent tube. The EE was changing a vent tube out when a small rock came loose from the top and struck them on top of the head. The cause of this incident was improper body mechanics / positioning. PPE was not a factor, mining systems was not a factor, compliance with rules and regs was not a factor, miner proficiency was a factor.
