Narrative

Employee used a hammer and a prybar to free a 10-12"" rock wedged in between C2 chute and belt. Rock shattered, causing a large fragment of the rock to bounce and eject back towards the employee. Rock made contact with employee's forehead above right eye. All proper PPE was worn during the time of incident. Employee received 6 stitches and returned to work.
