Employee was a miner bolter on the full face miner. The EE was standing beside the miner while cutting a corner when a piece of debris flew from the head on the miner striking the employee. The cause of this incident was improper body positioning. PPE was not a factor, mining systems was not a factor, compliance with rules and regs was not a factor, miner proficiency was a factor.
