Narrative

Employee was attempting to install a rib bolt. The EE was hit in the back with a small rock / coal. The cause of this incident was improper body mechanics. PPE was not a factor, mining systems was not a factor, compliance with rules and regs was not a factor, miner proficiency was a factor.
