EE claims injury (lower back sprain) occurred on 7/4/24. EE reported injury to company on 7/9/24. EE requested to seek treatment on 7/11. On 7/11 EE was taken to Concentra and received work restrictions of, ""No shoveling, may lift up to 50lbs constantly, and May push/pull up to 100lbs constantly"". On 7/11 the injury became a MSHA reportable.
