Narrative

Employee stated was cleaning around #1 Belt take-up, as EE walked inby the take-up EE stepped on a piece of 6"" discharge waterline that was partially buried in the mud. This caused EE to fall to the mine floor on right hip. Employee complained of pain in right hip and groin area.
