## Narrative

Unplanned roof fall above anchorage point discovered reportable that was approximately 45' wide, 30' long and 5' high. Fall appears to be from gradual degradation of mine roof, area was mined circa February of 1957. No problems with equipment or compliance issues, training or PPE
