Narrative

While using a lever chain hoist on the 4 West beltline (while installing belt) the lever slipped and they felt like they pulled something in right elbow. Started missing work 1/13/23 for lateral epicondylitis of right elbow. No problems with equipment or compliance issues, training or PPE.
