Narrative

Employee was on the longwall when EE was asked to get the acetylene tank. EE was struggling to carry the tank alone, so EE switched it from shoulder to shoulder. Setting it down and picking it back up, 3 times. On the last time EE picked it up, it slid off and EE heard a pop in shoulder, causing pain and numbness.
