The injured was drilling a hole to install a roof bolt on the 3 South Head Gate Panel, when a piece of rock measuring approximately 6"" W X 5-1/2"" L X 1/4"" fell from the top and struck the injured's drill pot and a piece broke off and bounced up striking the injured just above the bridge of the nose. Causing a laceration that took one suture to close.
