Narrative

IP was driving haul truck when they hit a pot hole and began to experience neck pain. Was assessed by EMT. IP went to clinic with supervisor for further evaluation. 03.04.25 visit ""Avoid neck movements that increase pain. Avoid overhead shoulder movements that increase pain."" Worker able to perform all normal duties at a slower pace.
