Narrative

Employee was bolting off miner when piece of rock came out of rib EE tried to guide it away with drill steel, it pushed EE causing EE to lose EE's footing. EE fell injuring EE's back. PPE was not a factor, mining system was not a factor, compliance with rules and regulations was not a factor.
