## Narrative

The crew was laying 2"" poly pipe, the end slipped, striking the miner in the face below right cheek bone. Employee refused treatment at the time of the incident. Several hours later it started to swell. Employee went to the emergency room Sunday (5/18/2025) as the swelling in the cheek would not subside. It was reported to me that employee suffered from a fractured cheek bone.
