## Narrative

The crosshead for the Single Drum Lining Hoist became caught on top of a concrete pipe bracket that extended from the shaft liner 8.5"" at 388' as the hook continued to be lowered down to a depth of 500'. The crew moved the work deck back to center the crosshead fell off the concrete pipe bracket and fell down to the headache ball causing 6' of damage to lower end of the hoist rope.
