Narrative

Unplanned roof fall discovered reportable on Main West Haulage at 36 1/2 block at approximately 8:45 pm. Fall approximately 20' long, 16' wide and 10' high. Area mined near July of 1967. No problems with equipment or compliance issues, training or PPE.
