Unplanned roof fall discovered reportable on 4 belt at 40 block at approximately 12:30 am on 08/24/25. Fall primarily occurred on the offside walkway at the intersection. Fall was approximately 20' long, 6' wide and 4 1/2' high. Area was mined near December of 1967. No problems with equipment or compliance issues, training or PPE.
