## Narrative

Injured employee was roof bolting on the left side of the continuous miner. Employee was in the process of installing a rib bolt when a piece of coal/rock fell from the brow behind them and struck them on the left shoulder. Was diagnosed with a deep tissue contusion and given time off. No rules or regulations were violated.
