At approximately 8:03am the bracket holding the top counter weight wheel failed and a piece of the bracket fell and damaged a sensor on the bottom of the sugar run elevator causing 3 miners to become entrapped on the elevator. The miners were safely removed from the elevator at 9:33am. No problems with equipment or compliance issues, training or PPE.
