Narrative

The employee was holding an 8"" HDPE pipe while the other employee was unbolting the steel flange at Pad 6/7 - pipe lay down yard. Once the upper bolts were removed from the HDPE pipe, the spool shifted, and the steel flange swung down, striking the left pinky finger, smashing and lacerating.
