EE was cutting piece of steel with 21"" torch out of the stilling well in 4 Tails Hydro. EE cut from the bottom of seam leaving ~6"" gap in middle, then cut seam from top down. The piece of steel shifted, broke off, and fell backwards over top of the rake making contact with EE left arm and face shield resulting in stitches to the nose.
