Narrative

Injured was cutting down roof screen to unload ""bagged"" material at 3 block on 3 belt with a grinder and some loose rock that was in the screen fell out causing a laceration to the posterior side of EE's left forearm that required sutures to close. No problems with equipment or compliance issues, training or PPE.
