Employee was in the process of starting the last row of bolts in the #3 face on the #4 unit when a piece of slate approximately 1'L x 1' W, 1/4"" T(Estimated weight of 6/12 lbs) fell from 6""-12"" above his head contacting the back of his head and back resulting in muscle soreness.
