## Description

On May 18, 2000, a Notice of Violation was issued for a Severity Level III violation based on a radiation protection technician deliberate failure to follow required procedures when he activated the alarms on the portal monitors a total of 14 times and failed to contact the radiation protection department before leaving the station.
