On August 12, 2003, a Notice of Violation and Proposed Imposition of Civil Penalty in the amount of $6,000 was issued to Mid American Inspection Services for a Severity Level III problem involving two violations, the failure to secure from unauthorized removal or maintain constant surveillance of licensed material, and the failure to ensure that shipping papers are in a vehicle while transporting radioactive material. On April 10, 2003, an NRC inspection identified that a radiography camera was stored in a vehicle being repaired at an automobile dealership for seven days. In addition, the radiographer had removed the shipping papers from the vehicle when it was left for repairs and the dealership personnel test drove the vehicle on public roads.
