What happened: the air bags failed to deploy during a crash. Question: What component or system failed or malfunctioned? Answer: air bags, automatic emergency braking, forward collision warning. Question: Is it available for inspection upon request? Answer: yes Question: How was your safety or the safety of others put at risk? Answer: driver-side air bags failed to deploy upon a rear-end collision with the vehicle to my immediate front @ 40 mph. Question: Has the problem been reproduced or confirmed by a dealer or independent service center? Answer: No. It currently is at a repair facility as of this morning and itâs apparent that the air bags did not deploy. Question: Has the vehicle or component been inspected by the manufacturer, police, insurance representatives or others? Answer: No, and not to my knowledge since turning the vehicle into the repair facility this morning. Question: Were there any warning lamps, messages or other symptoms of the problem prior to the failure, and when did they first appear? Answer: None. No previous warning lamps, messages, or symptoms. *NOTE: I have dash cam video of the collision in my possession.
