The contact's son-in-law owned two Pittsburg Automotive 6 Ton Heavy Duty Jack Stands, Part Number: 61197, Serial Number: 360271732. The contact stated that his son-in-law had put the rear-end of a 13,600-pound forklift on the jack stands and was working underneath the rear end of the vehicle. The contact stated that the rear-end of the Fork Lift weighed a little over 6,000 pounds. The contact stated that the jack stand underneath the left side of the Forklift seemed to have failed, causing the forklift to tip over to its left side and crush the contact's son-in-law. The incident occurred at the owner's shop. The Fork Lift was lifted with the assistance of a crane approximately 10 minutes later. The contact's son-in-law was transported to the hospital by ambulance, where he was pronounced dead at the scene. The medical physician advised that the owner's diaphragm and liver were crushed by the Fork Lift, and the owner did not take a breath after the impact and passed away. The contact stated that the Occupational Safety and Health Administration (OSHA) arrived at the scene and made the contact aware of NHTSA Campaign Number:Â 20E016000(Equipment), which the contact related to the incident. The contact then asked all the workers of the shop whether they were aware of the Jack stand recall, and all the workers confirmed that they were not aware of the recall.
