Subject: Vehicle Safety Defect Report – Brake Failure and Collision Mitigation System Malfunction  To the National Highway Traffic Safety Administration,  I am filing a safety defect complaint regarding a Honda vehicle that experienced sudden brake failure and a suspected Collision Mitigation Braking System (CMBS) malfunction, resulting in a crash on [date].
