WHILE TRAVELING 5 MPH AND ATTEMPTING  AVOID A CRASH CONSUMER DEPRESSED BRAKE PEDAL WITH FORCE TO FLOORBOARD RESULTING IN ACCELERATOR PEDAL BEING DEPRESSED AT THE SAME TIME. THIS  CAUSED A CRASH. PLEASE PROVIDE FURTHER INFORMATION.*AK
