EVENFLO MODEL # 4761196P1, DOM 12/14/2001. DURING A 65 MPH FRONT VEHICLE COLLISION THE CHILD SEAT DISCONNECTED FROM THE BASE. THIS CAUSED THE CHILD SEAT TO GO THREW THE REAR PASSENGER WINDOW. CONSUMER SUSTAINED INJURIES. *AK
